This was recent so maybe it doesnt work anymore.
In both cases, I had no issues at all to reach the dmg goals.
The item one was with Echoing Band alone and Spirit Warrior Dash with Shieldmaiden.
Also, the Ghost Wall in Viaduct can now only be used for "hit X things" achievements. It has had it's HP stripped. They were 1M HP with a 90% damage resist. Now they have 10k HP with 99.9% damage resist.
EDIT: and they have damage numbers forced off so you won't see damage dealt to them.
No, I have done all my runs today on the new map.
Devs, maybe word your achievement description properly next time?
And since it does more damage and have bigger aoe than normal dashes, dashes dont do any damage after you have chosen spirit dash. So you have to use vanila spirit warrior
For me items would be:
Consumables
Powerups
But not sure if gear/equipment are also considered items as well.
Other than that , I'm loving/hating the devs for this update. 😁

Item was for me always things to use as a tool or consumable.
Things I have on me are gear.
The whole lot in sum is equipment.
Older SNES RPGS I pllayed in english teached me it a bit this way,
although gear was often refered as equipment and tools/consumables items.
Not sure 5m dmg is possible with powerups only...

Good for you, but theer is something extremely off with the dmg numbers in the pause menu. I had 2.9 mio damage with the echoing band alone listed, yet the achievement didn't fire until way later, with way over 5 mio. Wasn't enough for the 5 mio damage achievment, tho.
Or items is something else entirely.
Yes, the wording for some of these quests needs to be clearer.
I played with the Landsknecht in the new map on agony and got all the technician achievements in one go (I was even splitting my attention with getting another achievement while doing so). I did keep my build light on abilities (only 1 or 2) to make sure my items had the chance to do damage.
*And echo band which I forgot to list
I took transfixion as my only ability, just to help with AoE in the early game.
