20 Million Damage "with Items"

Alright, what does this mean?
That number is way too large for things like Thunder Crown and Electrostatic Treads, or the random terrain objects that can burn/icicle enemies.

I just need to figure out a build that manages on Agony but doesn't kill things before other effects can.

Use Thornfists or Defiant Plate. Get some levels then just turn off auto attack entirely. With decent defense and regen you won't die. Especially, if you use reactive, everything else dies the instant anything hits you.

i.e. don't take any abilities at all

Are horde summons not considered items? Should be an even easier way to get it.

Got it by accident on Landsknecht with Echoing Band, while playing her Physical Projectile Build.
Should also be very easy.

AND I forgot I needed to be on Landschnekt. I went on Exterminator for bonus Area per level I think, since I forgot that key detail.

However, holy moly Gorgon Mask is insane. I got it over 70 million damage on its own.

Nah, Gorgon Mask (maybe since I took Cleric Mark and had high effect hit too) is nuts. I did it on map 1 Agony and until the very last few minutes, I spent most of the run easily dominating and even capable of standing still with my main weapon off and zero abilities picked up. I only manually fired my weapon at elites and bosses.
I did get an Agony version of Gorgon Mask with more base radius which helped a lot but still, this achievement will be extremely easy to clear in any run where you don't have weapons/abilities sniping Gorgon Mask's damage.

It's strongest with area + attack speed, use brokers cape to spread the slow. You are on the right track though, your plan is definitely one way to do it.

Turns out my initial bit with Executioner really was smart for this since the scaling Area was a big part of the Gorgon Mask going nuts.
Also map 1 Agony might be a bit easier than map 5 Agony.
Also also 2 million (and 5 million) is not 20 million so I really did go overkill.

Both hurdles were easy to clear anyway. As long as other weapons don't interfere, Cloak + Gorgon Mask can wrack up the achievement credit in due course. Easier if avoiding other weapons at all of course.

Now to clear #301/301, 18k kills on map 5.

If you use Echoing Band, try un- and requipping it at the start of the round.
Switching the Ring Slots from left to right or vice versa may help to.

Thorn-fist states it only works with melee characters + sorceress. Landsknecht is range.

The easiest way is equip echoing bands for it proc and use phantom needles ability.
