Its one of the new quests/challenges for the first area.

Had a wander around during my first 1.0 run and could not find it.

follow the blood trails, defeat enough mob at both ends then get your prize

I beat this before and now it seems like its bugged out. No blood trails leading to skeleton mounts.

Blood trails were fine for me. The issue is you just may not encounter the blood trails as you're walking toward either scroll on the map. I think they're pretty much always a bit more westward in contrast to the NE and SE scroll placements.

All of the boss weakening items start near the well, so check in that area for the blood trails.

I recall doing something way back when I first started this game, but I guess now there is a quest for it? I'll take a peek at this next time I play.

This is random.
Mostly 1-2 screens away from the Well.
Normaly you will cross them when walking towards the Hand of Greed or the Ability Scroll far off.

Is the "Trail of BLOOD" supposed to be completely black, like petroleum or tar? Because if not I can't see anything on the floor like blood and I've been circling the entire map.

nvm: the quest must be done in the first level, that's where the blood trail is. Somehow it the quest UI shows in every single level so it's confusing.

I think the confusing part might be that while the Achievement in question is in the right place, it doesn't tell you that said Achievement can only be done in for example, Haunted Caverns.

That's the downside of randomly-generated content.

Goto the link. There are screenshots which will show you what they look like.

I'll acknowledge they can be hard to see but, even though randomly generated, they have a zone they're generated in so milling about the starting area will run you into them 100%.
