Sage quest: reach Agony IV

Is there a bug with the sage quest to reach Agony IV in Chambers of Dissonance? It doesn't seem to work at all for me. Here's a screenshot showing I reached it, yet the quest not completing.

Hello there,

Agony V should unlock it and did you used Hastening Sands?
Some Sage Quests seem to interfer with HS.

I did not use HS. You're saying I actually need to reach Agony V?

Do you remember if you filled up the bar completely on Agony IV? Maybe that's what's needed
