Agony Levels?
I see some people says they reach level 100 in Agony. Is that your Character level or the Agony level? Because when i play it, the progression of Agony levels is pretty slow and i'm not sure i ever got beyond 3 yet.
Also can Agony levels only go up or also go down? (like a thread level measurement)

It is character level. Max agony level is IV.
Agony level can only continue to go up. The more enemies you kill, the more you fill the agony gauge.

Are you sure Agony is not going down? I just finished the Agony Boss in the first Level and Agony was max, but then went back to Agony 3 after at the end end i killed the last Boss

"Over time, the meter slowly increases.
1 Agony Rank is gained every 4m 48s (24m for 5 Ranks).
Reviving decreases the Agony Meter by 20%."

Update: You derank 1 agony lvl per revive.
-20% is from max lvl 5 so its -1 rank.

You're welcome, i just tested it to verify, i can confirm, that you dont need any kills and that it takes 4m 48s to lvl up with 0 kills.

Whats interesting though is, if you die and revive, it does not take 20% from your current meter. So it could possibly take 20% from the max meter at lvl 5 or has been changed with a patch. Could have a certain fixed value or something.

What i found is reviving at agony 1 with meter 0 resets you to agony 0 with 0 meter. So basically -100% from your current meter at that state.

Yeah it should be 20% from max lvl, which is -1 lvl per revive.

So if you revive you should always derank 1 agony lvl.

Everything in here has been accurate. I'm just here to add how it used to be and where I wish they would take it...

Old System
Agony didn't used to be like it is now.

Originally, Agony slowly increased, and living mobs would begin to drain your meter after a few seconds. So you had to kill them quickly to keep it going up at max speed. The slower you killed, the more the meter stalled. If you struggled to kill enemies, especially leaving elites/champions/bosses alive for minutes at a time, the Agony meter would decrease to 0.

This was what the devs called "performance based Agony". However, there was no end to amount of people complaining about never being able to raise their Agony level to 3 for achievements.

Old System Feedback
It was so frustrating to hear players whine about the Agony meter but I understood what they meant. The old system did not "reward" good play, it only punished bad play. That is, no matter how well you played, how fast you killed, the best you could achieve was to prevent the meter decreasing. All you did was avoid a negative. So the best you could hope to feel if you played well was "not bad". Meanwhile, everyone that played anything less than perfectly felt awful, because the meter slowed down (or stopped and went backwards). Hence the issues. So now we have the current system. Overall I think it is an improvement because all players feel better about it.

Potential New System
I really want performance based Agony to return, but without making players feel bad. I really wish the player was in control of Agony increase instead of only controlling the decrease. That is... Agony does not increase at all (by default). It only increases when you kill mobs. Kill more/faster and the meter goes up faster. This rewards good play and does not punish those that are slower. Those that want to chase highscores (me) can push for the fastest kills, meanwhile those that just want to get an achi have the whole 30mins to get Agony 3. Win/Win.
