HoT's effect opacity slider alone makes a gigantic difference. Late game Vampire Survivors often turns into a black screen and/or screen of rainbows filled with damage numbers. And that's just using 1 skill + limit break.

Besides the visual clutter in VS, it just never got me hooked that bad.
Played the VS DLCs latley and it just felt like a chore to me.
HoT on the other hand, there is nothing left to do, but i always come back to do another stupid run for fun.
imo VS is still the most feature-complete and meta-rich of the survivor games, which is logical given that it's the originator, but I find myself enjoying its 'imitators' just as much, if not more.
- Halls of Torment is easily the most atmospheric out of the bunch.
- Rogue Genesia is a great one as well, offering an insane amount of ability customization, 'campaign' progression and short burst playsessions instead of mandatory 30 min runs.
- Disfigured has a very original concept built around the player's line of sight - plus it's free, definitely give it a try.
- Deep Rock Galactic Survivors demo was very enjoyable as well. Barebones, but still a lot of fun and very original with emphasis on mining.
Vampire Survivors however has one very important feature - coop. I hope more survivor-likes implement it.
I would mention Boneraiser Minions as well. Very unique game built around survivors idea and while i think I like HoT bit more than VS, im not sure whether it is a BM that i would choose as the best of. Though, it is hard to say. Good survivor games are good because they are not mere copies/clones, but because each offers something different. So I guess BM, VS, HoT deserve being top three in genre, with 20 minutes till dawn bit behind, but still very good and worth playing.
Knew about 20 minutes till dawn for a while, maybe I'll get to that one as well.
I do highly recommend Rogue Genesia though - if not right now then at least when it gets out of Early Access
I've played RG ages ago - actually a free demo if I remember correct. It was okay, but didnt offer anything interesting above just walking and killing.
BM - I highly recomend. I really enjoyed playing it. Just one thing there might be bit annoying - visuals. They look very oldstyle'ish and you have to spend some time in the game before you start actually seeing things ;-P. But even then there are moments when u play blind... but it is worth it ;-P
Will def try it after I'm done with HoT, which is soon from the looks of it
Not a single stage, or challenge, or anything contains any amount of difficulty in that game.
