Summon Duration

I just had a run with warlock where I was wearing the summon undead ring and the increased summon duration 50% ring. I took every single duration buff i saw every chance I could. My question is, how the hell do you get more than 3 summons from the ring? It says it is supposed to be able to summon up to 5 and that you get more summons with higher duration, but I never saw more than 3 at once (usually no more than 2). Am I just misunderstanding how this works or is there a trick to it or something?

Edit: since the patch that improved summons, these seem to work fine, but I'd still love to know the specifics behind its functionality.

i started with 2 rings:
Necromancers Clutch: every 15s spawn a skeleton. Can handle 5 summons (item description).
Demonic Blood: every 5s spawn an imp. Can handle 5 summons (item description).

but all i get is 1/5 skeletons and 2/5 imps... :-/

edit:

i upgraded summons attack and movement speed +10%...
now i got 3/5 of both.... oO !?

attack speed and projectile/attack amounts. alongside duration. ive had 7-9.

I had even 7 skeletons and 7 imps, so just make the right build ;)
