...and screaming "broken!" into the void doesn't help either. Perhaps you can give some details? How is it broken? What did you do in your attempt to achieve it?

More often than not people come to various game forums screaming foul when what is actually happening is they didn't do the thing correctly. Without DETAILS of your experience, scepticism is a fair default position.

Remove everything that increases your base hp (equipment/blessings), dont pick the vitality trait.
Pick all 4 Dedication - Bloodletting Traits

pretty sure it is i dont remember the quest names
but i can tell you what to do anyway:

the only thing that matters for this quest is that your maximum possible HP NOT your current HP pool is a certain amount, it wont do to just take dmg up until you run around with X amount of HP

So i dont know if its working if you can keep your hp under 250, but it clearly says maximum, so simply reduce your maximum under 250 with 4 Dedication -Bloodletting.

You have to drop your Maximum health down below 250, easy to achieve by taking Bloodletting buffs, Was able to get down to 34 max HP by doubling the first 3 with potions. Getting below 250 was doable with just doubling the first 2 (174 health). Then just survive 5min

On the first try I only got my HP below 250, but not the Max HP, so it doesn't work. You really need to lower the Max HP to 250.

I already unlocked it, then after using a revive on the boss it was gone after finishing the level and now i'm unable to unlock it again.

I just got the achievement right now with 244 max HP. The quest clearly states less than 250 MAXIMUM HP.
