Change this setting: RealToneCableOnly=1
It seems that this make the game only accept the RealTone Cable for input, and the plus side is that's the only input device it ♥♥♥♥♥ with when calibrating gain.
This is a common Windows quirk. It will reset the mic level to the default gain) usually 17 when you switch applications. unchecking the "allow applications to take exclusive control of this device" might prevent that, but you may find other apps will have issues using the mic if you do that.
I've simply gotten into the habit of checking and adjusting the mic gain while the game loads and don't worry much about it beyond that.
FYI and pro-tip - RS will set a higher mic gain if your instruments input level (read, your volume knob" is set lower. but this can cause problems with note detection.
