On the Nvidia control panel, display > Change resolution, make sure the resolution you have selected is on the PC section, and has 144 hz.
A windows update screwed mine and I was capped at 60 fps, which generated a whole lot of lag spikes and stuttering.
A friend had the exact same issue too.
There's no blatant "display" option in the Nvidia control panel.. that I can see
I've had the same problem, it's coming from the cpu, as the game is so easy to run, cpu goes into a low powered state and sometime just gos back to normal while playing. This cause micro stutter. It is resolved by setting windows to high performance
Here's something I noticed that hopefully helps you solve your issue: after reading your thread, I've started the game out of curiosity, and while playing with it for a bit, quitting and starting again, I've noticed the game suddenly ran at exactly 50 FPS for no apparent reason instead of the usual ~63. This persisted across Steam restart and a Windows logout. The solution was really weird. I had to shift-tab into the Steam Game Overlay, and close the Friends & Chat window that was apparently open there by default. That made my issue go away. I have only a few Steam friends, and it is my theory that Steam UI redesigns since the launch of Super Hexagon have led to this issue. Maybe people's animated profile frames which show in the Friends & Chat window when they're online are being drawn and animated even when the overlay is closed, and are causing the game to lag. It could be you're experiencing something like that, too.
By the way, if you have a double monitor setup, maybe you'll observe the same thing I did: the game seems to run just a little bit worse if it's located on the same monitor where you have your Steam Library window open. It's a slight difference of only 1-2 FPS, but noticeable nevertheless. Super Hexagon is really finicky like that for me, for some reason...
BUT, this is only true if I have the Steam main window open, NOT minimised, AND located on other monitor than the one I'm running Super Hexagon on. **AND** it has to be displaying either the Game Library (but NOT the home or collections view, it has to be one of the games selected) OR the Store. If these conditions are not met, opening and closing the Game Overlay will not work and the FPS will be fixed to around 50. To add to all that, this happens regardless if the Steam Game Overlay is enabled or not.
Yes, I know this is extremely specific, and you would be excused for thinking I was trolling, but this is really the issue I'm having. It might not be the same as the one you encounter. My framerate drops don't go below 50 FPS, unlike the 20-30 FPS you mentioned. Furthermore, I've never heard of anyone have the same problem as me, so I don't even know if this is something specific purely to my system. I should make a separate thread for my issue I guess. But, just so you know. You could check if any of what I've written helps, if you're desperate. I'm running Windows 10, by the way.
EDIT: Also, this might help. I don't ever use it, but I went to Big Picture Mode just now to test, and run Super Hexagon from there, and it seemed to run great, I didn't even get any micro-stutters. That mode does not have an FPS counter though, so I can't be sure, but just judging by the smoothness, the game seemed to run very well.

I'll be honest, I didn't know that steam had an FPS overlay XD but I tried it out and I don't think it gave me the full story. It was giving me a reading of an average of 60-63 fps but I don't think that is quite right.. it would be really handy if it also was able to show the 1% lows. These micro-jitters I'm getting are very small but very frequent.
My game does not look smooth at all, I was estimating 30-20.. I'd say 40fps would be about peak, but definitely not the smooth 63 fps steam was displaying!
I tried these methods you suggested, even going into Big Picture Mode, but I didn't notice any performance improvements.
I'm playing this on a gaming laptop (GL65 Leopard if that's important). No idea why this issue is persisting, but I just can't seem to manage a smooth game.
