EDIT: I completely forgot this includes the DLC too, and I'm not sure how much more it adds but I'm assuming a pretty sizable chunk.
I've played the game on ps4/ps5 and my last run on the ps5 I put in I think around 24 hours iirc and had JUST got to the second part of the game which is the piece of land just north of the starting area. But I also went and completed EVERYTHING I could in the starting zone (which is still pretty big) from farming flowers to get different outfit colors/configurations to different armors. Finding all the vanity items etc. I ended up going to the second zone with fully upgraded sword and dagger and pretty substantially upgraded bows, abilities etc.
Just depends on how much you enjoy the game and want to play all the side stuff. There's also NG+ as well if you are into that kinda thing too PLUS Legends mode you can sink even more hours into.
To answer your questions, this game is pretty big. Not too big that it's unfinishable, but enough that it makes the price tag worth it. If you take your time, and try to go for everything, 80+ hours in a single play through. Side quest are not too long, but they are very good. Collectibles and exploration keep you entertained if you need a break from the story. I've played multiple play through on the original PS4 and then on PS5. Always go for 100% on each. That's what I'm basing the playtime estimate.
It adds a brand new area (Iki Island) with it's own collectibles, quest, and story.
