Flash of Steel (Perfect Parry) achievement
This achievement requires you to kill 20 enemies with a counterattack after a Perfect Parry. However, as I approach the very last story quest of the game, I feel like I should have gotten that achievement by now.

I'm playing on KBM and from my understanding, Perfect Parry occurs when you parry (ALT button) an attack just before they hit you. Time slows down (indicating your parry was perfect) and then you can launch a counterattack with a light (LMB) or heavy (RMB) attack.

If that's the case then I can't count the many times I have killed an enemy with a counterattack right after a perfect parry,

Am I doing something wrong, is the achievement bugged, or am I overestimating the amount of kills I think I have?

FYI. I'm playing on Hard difficulty and I am using the Mizu-no-Kami charm which increases the window of perfect parry.

I think you got it mixed up. I am a KBM player since 2000. In G.O.T., perfect parry can only be achieved using the Q button when the enemy attack. You will know that you have perfect parry successfully when you see the opponent sort of opening both his arms when you parry his attack. And then you kill him. Of course, if the opponent is strong, even after a perfect parry, your following attack may not kill him. Like for the brutes (huge dude with shield or big sword or spear, or fully armoured generals). For these type of enemies, you will have to "hurt" them a bit and hopefully, you will get a chance to execute a perfect parry on them and then successfully killing them. So, it is a patient game as far as this achievement is concerned, well, as far as during your first playthrough.
By saying that, if you enter into a NG+ game, by this time, you will have upgraded your Jin severely, stronger, fully upgrade katana and/or armours. You will be more likely to kill a strong opponent by executing killing blow after executing a perfect parry.
Left ALT key is for executing a perfect DODGE, not perfect parry. Hope this help. Good luck and have fun :)
Oh yes, and ah, keep cool and be patience.-

for those kind of achieves just go to a map and do that over and over
on legends mode you can purify gear with some random actions, one of them is to kill enemies with perfect parry or perfect dodge so its suppose to register w/o trouble
