it adds new stuff that you can only get with new game+ other than that everything is the same
It adds a vendor name Baku the Voiceless with more cosmetics plus you can make your weapons and ghost weapons stronger and armor stronger.
It also allows one the ability to cancel most cut scenes and eliminates the entire intro and all tutorials. That alone cuts about an hour's worth of time wasting stuff that one doesn't have to go through again to start a new game.

However, the downside is that one also starts with all the upgrades that they finished the original game with. That essentially eliminates much if any progression. I think they should have offered the game+ selection with a choice of either all upgrades or starting from scratch. If there was such a choice on the new game +, I missed it. Otherwise, one starts with zero arrows, knives, and bombs but that only requires getting some supplies and a visit to a trapper to stock up.

That's how one sees all the videos on YouTube of players dominating early missions
fully upgraded with all the techniques and weapons.

New game+ is more challenging, enemies are a bit harder. But you can get new armor and masks and so on.
In a new game plus, you get to keep all the armours, swords, all their upgrades, skills, that you have obtained in your first playthrough. But these will be available to you after the prologue. The enemies are stronger and hit harder. I mean you can die after one hit from a low level minion. Some players complained that this is too OP but with the skills and armour fully upgrade or not, you can really string together some impressive fighting sequences. This provides tremendous self-satisfaction. And if you have the DLC, Iki-Island, well, the enemies are much harder. But by now, it would be a walk in the park facing them. If you want more challenge, just never engage in Stand-off.
