It's just like pretty much every other open world game out there. What is it that you think makes the open world in Witcher 3 any different from the open world in Assassin's creed, or Far Cry, or Red Dead Redemption, or GTA?
You play through a tutorial of sorts before you're let loose in the open world. To begin with you're limited to the southern part of the island in act 1, the center part of the island opens up in act 2, and the northern part opens up in act 3.
You can also quick travel to pretty much any location you've previously visited.
They also did a good job with not cluttering your UI with a bunch of markers. You can use the map and set waypoints, or you can just explore and look for visual cues like birds, butterflies, smoke, fireflies, etc, to guide you to points of interest.
I will say this game has the best sword combat I've ever seen in any game by a considerable margin.
