If that is the case, switch your audio to "bookshelf speakers" or some such setting, in the game. It fixed mine. I didn't have to fuss with anything else, as seen below. Try some/ all steps. Your mileage may vary.
Others have had to lower their audio quality from STUDIO to DVD, and others (still) have also had to disable AUDIO ENHANCEMENTS.

1- Option-click (I'm right handed, so my RIGHT CLICK brings up an options pop-up) the "speaker icon" in taskbar
2- Select-click SOUND SETTINGS (Again, as a reminder, I'm right handed, so my LEFT CLICK confirms actions)
3- Select click your device/ devices under OUTPUT. OUTPUT should be the very top option category. When the "page turns" and you're presented with new options, look to the bottom of the page for "AUDIO ENHANCEMENTS."
It's likely set to Device Default Effects.
Select-click on the name/ drop-down arrow and select-click OFF.
Had the Bookshelf Speakers not worked for me, in the game audio settings, I would have had to come here and change my "Headphones" AND "Headset Earphone" AND "Speakers". However many output devices you use, you'll want to check each one and turn off AUDIO ENHANCEMENTS. This step worked for my friend/ the Bookshelf wasn't enough to fix the sound effects cutting out.
We think it has to do with entering/ exiting "Hearing Mode" (enemies highlight in red, interaction objects in white, etc.) and the game not properly adjusting. It's a port... so yeah, they can never just work.

3- Scroll down the pop-up page to the bottom and select-click MORE SOUND SETTINGS
4- In the new pop-up window that appears, titled "Sound", locate your OUT/ OUTPUT device
5- Next, option-click whatever OUTPUT device is used to deliver sound to you, and select-click PROPERTIES
6- In this new window, select-click the tab at the top of this new window that is titled ADVANCED
7- Select-click on the mini drop-down arrow that is located just to the left of the TEST box (it might have a green colored, typical, media "play" button icon)
8- In this drop-down list, select-click the follow:
----- 2 channel, 16 bit, 48000 Hz (DVD QUALITY) -----
9- Select-click "OK"
10- Profit
Hopefully, if it came to you doing this after the Bookshelf Speaker selection in the game audio settings didn't work for you, nor did disabling AUDIO ENHANCEMENTS.
If none of this helps... !@#$%^&*.... I HATE ports for THIS reason and feel for ya, I really do. It's ALWAYS a crap shoot with console ports.
Fixed it for me where nothing else would as i am on the Iki Island DLC.
I've heard this from other people, so I wonder if it's something to do with having that questline active.
After killing some mongols in a camp i have my sound again.
So hopefully this patch is going to get it fixed.
-if you have your headphones connected to your phone's bluetooth as well, turn that off and it should fix it, if not you might be out of luck.
Thankfully it's fixed in this last patch update!! Praise devs!
