Quite a lot has changed - the big thing that will benefit you as a returning player, though, is that the tutorials are now actually useful. I'd suggest you start with those, they walk you through the basic gameplay (probably not much new info there, but it's good to get the wheels turning again) and give a very brief rundown of more advanced mechanics.
Aside from that, the big changes are:
Heroes now need decorated rooms, there's a list of which decorations will attract which heroes (search up "hero requirements" and one of the posts should come up). There are also a couple of new heroes (Sips is the inspiration for one of them, and Vechs inspires another), plus a host of small changes and improvements to heroes. Take note of the Shrine item, it can be used to "summon" a new Punchwood, Herbalist, Sips or Vechs hero if you have one that has died.
Townies and heroes have been given "level limits" that prevent them from going deeper than the level you set - great for stopping the townies from hauling items out of the dungeon that heroes are still clearing. You can set these limits from the townie and hero panels respectively.
Buried towns have been added, not sure if you'll have seen them previously... basically, there's a ruined town buried in the middle of the dungeon. Inside are many many enemies, some traps, loots, and some useable furniture and building blocks which you can "recycle".
Caravans - again, you've probably seen them in Sips' LP, they're pretty simple to figure out; all they need is a market place.
Fluid mechanics have been added so you can build with water. 2 new crops need to be planted close to water, and there's a "water elevator" to carry water upwards. There's also a "fluid blocker" attribute that lets walls block water and lava, so no more game-ending floods (unless you happen to have built entirely out of logs and hay...)
Finally, the big one: terraforming. You can now rebuild the terrain, using either natural materials or monster drops. Combined with caravans selling items from "off-map", you can now have access to every resource in the game on any map.
Enjoy getting back into the game, and if you have any further questions, don't hesitate to ask them. I'd suggest you quickly search the recent topics, because a few of them have cropped up (and it might save you some time if they've already been answered), but we're happy to help if you ask.

A small note, in last patch scaffolding decays, so that means you dont need to delete them, but it also mean you need to finish building in several days before they are destroyed, so dont do a huge project at once.
Barbarian - bone bed
Elf - harp
Knight - wooden bed
Dwarf - room to be built underground
Mage - wooden cupboard
Sips - necromancer statue
Vechs - bird in a golden cage
Punchwood - a wooden bed and a stone male statue
Herbalist - Potted plant
Rogue/Thief - Decorative Chest
Highlander - Room above level 5
And why this list of hero requirements isn't pinned when it is asked about once a week is beyond me. I guess we are just keeping that sense of wonder and confusion going for the new players.
