Forge recipes?

I don't know if I'm just stupid or if it's not working but I don't know how to get forge recipes. I've just been reforging all the basic gear you find/loot/buy but I want a new kit for one of my dwarves.

I can find recipes via a google search. But I tried doing one, assassins head glacial head white reaver head, didn't net me what it should have. It turned into a wizard hat. They were all the same rarity, proper order the wiki showed as well.
