Where are the difficulty settings NOT in Options

Where are the difficulty settings ?
I have been looking but canot find them
I have searched here for difficulty settings but no luck.
Going to check local cache again for missing files.
Anyone point me to Difficulty Settings ?

I don't think there is a way to change them once you begin the game.

I have nothing below Grenade Indicator ?

Used down arrow to look even tried mouse wheel, Nothing, tried going up too.
I donrt see a way to reset the game when running it or in properties.
Tried starting from steam here and desktop icon.
Local file verify integrity finds 1 file missing 1.3 kb gets it no change.
I think I have a backup copy I could use to reinstall hopefully the copy is correct, my DL speed is 1 GB / hour
Sent Far Cry 3 in My Games in documents to Recycle bin.
Something is definitely missing.
Thank you very much and more advice from anyone will be gratefully accepted.
Edit: could backing up to the cloud here be the cause ?

Completely deleted ubi spyware, all traces.
Reinstall ubi all seems to work.
Start game still no settings below Grenade.
Check far cry local integrity 1 file still missing will be reaquired,....Not happening.
giant pita.
Really wanted to play this, was motivated.
Delete all far cry and Ubi content, will reinstall later date this might fix it.

Well I have reinstalled the game and followed all the advive I have found.

So for laffs I ran the malware programs Hitman Pro, JRT (malwarebytes) Malwarebytes full version, and a few others including the cloud.
ALL report Farcry infested with Malware most noted was ubisoft and punkbuster.
I am sorry, I do not want anything to do with Malware.
Will Remove.
Edit: sql injection Seriously ? also I ran the malware stuff before reinstall = Nothing found.
Now I get to spend days cleaning up the mess.

just a little note:
If you open the game and go to Gameplay, then Difficulty wont show up. You need to start your game and actually load a save file and then it will show up oder Gameplay.

Let me see if I got it right.
I need to laod a save file but to do that I would have to play the game to make a save file ???
But I can't save anything because I keep dying before there is anything to save.

I dont think there is a save file at install. But isnt there an Option when you start a new game where you have to pick a difficulty?

I think the malware, PB is due to it being DRM and stuff, I knew that couple of years ago , cracks of new games were also very often marked as malware, but were "clean", so false-rights. Dunno if thats still, and in this case, also the case, Im not using cracks anymore.

Edit: so I just checked when I made a new game I was asked which difficulty I wanted to play in.

There is suppose to be a setting for difficulty but I have downloaded it fresh after purging it from my system twice and I have nothing below Grenade.
I know it is not due to a save because each time I install the game fresh it goes back to the very begining like when I first bought it.
Baffling.

Reloaded from scratch a week or two ago.
I just ran Far Cry 3 reached first check point.
I was finally able to check and it is now at the esay setting.
Thank you very much to all for the help.
I have reached the rock wall after the deer through the tunnel of rock, then the man / then Bear ,killed me.
I have not figured out how or where to climb the rock face / vines yet tried space and qwerty etc.
I'll get it tho.
