The "fade to black" is similar to what happens if you lose the buggy (eg drive it off a cliff), so it may have something to do with where you park the buggy before going to the bridge.
It's a good idea to keep several different saved games as you play. Some players also experience a bug in chapters 10/11 that has the same reload/restart solution.

It's a shame the OP deleted the screenshots showing the location of the bug.
The solution there is to use cheats to get to the map.

It seems like that there is a problem with the savefiles, not the gamefiles.
Noclip to the end of the bridge then spawn a buggy with the command "impulse 82" the load trigger in the tunnel should then load
I'm having this problem when I open the door to start messing around on the bridge right now, so the bug is definitely still crawling around. I've tried going to a previous save, but that didn't work. I tried going back to a previous save and moving the buggy, which is nowhere near the edge of anything and is in no danger of falling, but I'moved it anyway. The bug still happened. I don't think I could stand starting the entire level all over again. The bug might happen again anyway. I'll keep looking for a better solution.
restart
It is similar to starting a New Game, but puts you at the start of the current map you are playing rather than the start of a chapter.
Note that these two methods will reset any progress toward "counted" achievements (One Man Army, Bug Hunt, Counter-Sniper, Atomizer, Hack Attack!).
Use a save file if you want to keep your progress from the current playthrough.
Okay, thanks. So if I just save the current game before I do the "restart" thing it will not wipe out my saved games? I haven't gotten any of the One Man Army, Bug Hunt, Counter-Sniper, Atomizer, Hack Attack! achievements yet. So restarting will not have an effect on the achievements I do have, right?
