I had some issues too. I would recommend having an icon with the same resolution less than 600 pixels across. My mod that I recently uploaded has a 600x600 icon image in png. When I tried uploaded my original image, which was a 1600x1600 jpg it failed. Maybe this is your issue with uploading? You should also make sure the file structure is right. It should look like this:
AddonName>materials
AddonName>models
AddonName>maps
-etc
With AddonName being the addon base folder with all it's contents inside and is what you select for content folder during uploading. I did not try using vpk so I don't know if that works. If not, you might have to extract it into the base addon folder.
*Edit: Even if the upload fails, you might need to check your workshop as it seems that failed uploads create a blank workshop page, which while not doing anything bad ends up cluttering your own Hl2 workshop page
