I can't get "Trusty Hardware" Achievement?

Not even sure how I accomplished this, nor how to even fix it, but even with 76% of achievements unlocked to date, I can't get what you'd think is one of the easiest achievements?

I've got the same issue with "Defiant" as well-- performing the proper action, picking up the crowbar for Trusty Hardware, or throwing the can at the guard who says to pick it up and throw it away for Defiant; neither achievement is EVER granted for me, no matter how many restarts, loads, or new games... anyone know why these, and possibly others, would ever be unachievable?

During a seperate playthrough, sure-- but considering that sv_cheats usually isn't on by default, it shouldn't affect a new game after completely closing and reopening the game and starting a new game... should it?
