Bug list ive personally found

this is not comprehensive nor do i really know whats going on with all of it but i will label what ive seen

1. shaking the gym can too fast can cause the key to clip out of bounds
2. challenges seem to be scuffed sometimes? stuck in the dark or in messed up first person
3. skins dont seem to work? or they might be confusing to activate
4. on the roof of the building by seven and sons, i somehow managed to clip out fighting the enemy and had to reload
5. during ALTERNATIVE, if you fall on the lower path in the minigame it doesnt reset

thats all i found so far!

Camera remaining outside the room where Becky entered to, happened to me when I entered the friends puzzle room in Wendigo's mansion level.

you can softlock the game at the "no hostiles" lift by walking in and out of the lift right before it closes, when you fall off the ledge you respawn right behind where the lift is and into the void forever and ever. you cannot quit the game while perpetually falling, forcing you to force quit the game itself

I can't use the skins either, the challenges show me a black screen and the Ex Hard Mode shows me a blue screen. There's also a blue medal that was uninteractable, it's in that endgame area where you have to find an eye to make some sort of machine work.
The other bugs i've experienced on launch day are gone now so that's good.

I'm unable to pick the red cap after falling down on the transit project 01 stairs (with a pile of barrels below), not sure if it's a bug or I'm doing something wrong.

I started a new run on Ex Hard, died to the first boss because it didn't drop any bullets, reloaded the save and noticed the "Ex" mode got turned off becoming the regular hard mode, the pickups were back and pills heal only 1 HP. If you want to beat Ex Hard mode right now you can't die or quit the game.

Edit: I tried again, it seems like it's impossible because the first boss will never drop more bullets and the bat is useless.
