Keeping your theory of the connection between Hooker and 50 Blessings, I would be more likely to agree with this connection being already broken when you find her, that's why she thinks you gonna kill her, she betrayed the Janitors BEFORE knowing you. But anyways, I'm not very sure...
As for the mask, the name is pretty obvious, refering to romance, but the horse... well, as Don Juan is spanish, may we should translate 'horse' for 'caballo', which in Castilian is used as slang word for heroin, possibly the drug she was injected with when you find her.
