Don Juan Mask Permanently Unavailable

So I downloaded Hotline Miami during the last Steam Sale. I love the game and I'd be playing it a lot more, but I got caught up trying to fix an issue where the Don Juan Mask doesn't become available after beating the appropriate level.

Strangely when I let my friend play the game at his place (on his PC) he unlocked the mask (this was the first time the level had been beaten). But I didn't take the save file from the computer he was using because I wanted to beat the levels he'd played myself.

When I played the Don Juan level back home on my own PC I unlocked a weapon he had also unlocked in his play-through, but the mask itself didn't become available. I've played it again several times even starting from the beginning and nothing.

Was this issue caused by letting him play the level first on a different computer? I can't see how that would effect anything as I didn't use the save data. Any thoughts?

Masks are unlocked when you do a sufficient highscore on a level. If I recall well you need to get a C- or better to unlock a mask tied to a level.
