The scene is built up (music, visuals, etc.) so that it culminates at that particular point, and the hard, non-avoidable, direct question hits you like a truck and makes you question your sanity for playing Hotline Miami.
Hence why it stuck with the game, just like "Gotta get a grip" for HLM2.
In my opinion, that question was epic.
In every level you go around murdering and brutally dismembering people, the music and neon vibe hyping you as f*ck, and then it brutally stops, there is an annoying siren-like ringing sound (just like a "hangover tinnitus") and you are forced to walk back to the exit, with the bits and pieces of your victims lying around. This is where you realize you are a godd*mn feral animal.
The "disturbing vs. addicted" contrasted atmospheres have been discussed many times on this forum: it is a pillar of the game's story as you're never quite sure if you what you're doing is real or if it's all just a metaphor for yourself, which would mean you're a psycho. The game is made to disturb you and it is completely normal to want to play but at the same time be repulsed by what you're doing.
I recommend you play the game and experience it for yourself, it's quite something :)
Nah, it's pretty trippy what they did - that trailer is not a trailer for the game, it's a trailer for Midnight Animal, the in-universe movie from Hotline Miami 2 based on the events of Hotline Miami 1.
