P.S After replaying the event a couple of times (trying out the different options) and what text you get, i believe this statement was wrong.
"With a pop and a sharp crackle, something hits you in the abdomen - and you are consumed by the unbearable, gut wrenching pain."
Sounds like it might to refer to being stabbed. You also have a wound when you then wake up in the vault.

Any mention of hearing the voice in the DLC after burning it out of you or tossing the shard is likely a bug.
I do not think so. If you agree to help him you never get the text saying how the entity was burned out even if you go with the dogmatic choice later. Its only if your resist him in the vault and he stabs you once that the text about the entity appears.
I'm probably misremembering the last time I went along with it or got a bug. I usually can't resist insulting him.
Regardless, hearing the voice in the DLC when you shouldn't is definately a bug.
