I am playing without toybox though, so I am not seeing the points until I click the options.
I did pick most heretical choices, but not all - although those I avoided did not carry many points. I even sacrificed Foulstone, which prevents me from doing the Dargonus level 5 projects, for the +15 points.
My dogmatic companions warned me shortly before I hit 300 points, and left me when I spoke to the inquisitor on Footfall. I get Heinrix and Argenta, but why the heck does Yrliet leave? Even Heinrix is kinda strange...shouldn't this half-heretic stay and watch me for the inquisition? At least I got the obligatory ominous threat from him, so I suppose I meet him again on Epitaph.
I cannot list where I got the points, because I kept no record, but as a rule they became higher with each chapter. But I think the +20 choices on Footfall are the highest you get.
@Higgy: you can see your points when you mouse over your next heretical rank.
I also had the companions warn me before getting to 300. Is just sad I need to do an entire other Heretical run to get zealot and complete that side of the story.
@Morgian - Can you see a breakdown of points? I know I can see 298/300 but I doesn't seem to show everywhere you get them.

I did not know it would cost me the colony permanently, since I had not chosen these decisions before, and I could not go back on grimdark. Later I hoped I would get it back somehow, since the quest to create a colony is still open, but that seems to be a vain hope. Which also killed the Dargonus tier V projects.
I just got to kill the spacewolves instead of the wordbearers, which is a fun variant for heretics. And I finally found out (at level 51) why people are so keen on being psykers. Must be the sword I cannot wield, and I play a bladedancer :(
Finished my heretic run now after taking control of the 'weapon' and taking over the expanse....for a while, at least...:)
Did you try sacrificing one of your companions to the black void thing? should give you 5 points. There are 5 heretical points iirc from the end of heinrix companion quest too.
There is also 5 points you can get from Uralon if you didn't bring Pascal or Argenta with you to listen to the word bearer sermon on kiava gamma(act 2) in act 5 by reciting the sermon to him while hes on your ship. You also get like potentially 15 points depending on choices from bringing Marazhai with you out of Commoragh and his resulting scene after first void jump. There are also at least 20 heretical points in chapter 4 for genestealer content right after the scene you can make the dogmatic no heal vow for the last ship genestealer event iirc. Something with some random group of traitors a few steps past.
The Act 5 one isn't going to help with unlocking the extra chapter 4 dialogue, but the first two I mentioned might. Most of the points in chapter 4 are locked behind rank 4 dialogue on both footfall and quetza for all the convictions. Chapter 4 otherwise doesn't offer points anywhere else that wasn't available in chapter 2 if it still would be as far as I'm aware. Chapter 5 has no points (heretical) besides the Uralon one I mentioned as far as I can see anywhere.

I don't have the DLC so none for me from GSC. I had pasqal with me, so I guess I missed that bit. And I didn't get any from Heinrix quest which I was kinda hoping for. Just sad =( next run through I might try even harder. But it may be a while, just gonna finish this one, so at least I don't need to do it on Grim dark mode =P
