It told me to put the png in portraits/0001 so I did that hit refresh and then it told me to put it in portraits/0002 and so on and not showing the picture I put in there

I have a different problem. I am able to put portrait images into a folder and the game seems to recognize that they are there. It even allows me to select that custom portraits to use when creating a new character. However, it does not actually use it. Instead, the game keeps displaying the last game portrait that was selected before I selected the custom portrait.

So, it seems like the whole custom portraits system may be bugged.

Edit: I went ahead and finished character creation anyway, just to see what would happen. To my surprise, the game did in fact use the custom portrait in the game, even though during character creation is was still displaying one of the game portraits after I selected the custom portrait.

So, you might be able to get it to work, like this:

● When on the Appearance tab of character creation, on the portrait step, click on Custom Portrait.
● If you have no empty portrait folders, click the box with the "+" in it to make a new one.
● Click one of the empty portrait folders (or one with a portrait you want to change) to select it, and then click "Change Portrait".
● This will open a new window; click "Open Portraits Folder" to open the folder that the portrait files will go in. (* See additional comment below.)
● Move the Fulllength, Medium, and Small versions of your portrait into the portraits folder that opened (replacing the guide files or old portrait files there), then close it. This should take you back to the game, but if not then click on the game on the task bar or alt-tab back to it.
● Click the "Refresh" button. You should now see your portrait in the game interface. Now click "Cancel". (It's weird, but don't worry, your efforts won't be canceled, it will just take you back to portrait selection.)
● Now click the portrait you want to use for your character from the custom portraits list. There should be a green checkmark at the bottom of it, which tells you that it is selected. Don't worry that your portrait doesn't replace the portrait above your character's model; that's a bug that won't prevent it from appearing in-game.
● Finish character creation. Once you are in the game you should find that your custom portrait is being used (provided the files you supplied are the correct sizes and in the proper format, of course).

(* At this stage there is some text at the bottom telling you "blah blah portraits/0001" or "blah blah portraits/0002" or whatever. This is what was confusing you. It is just telling you the name of the folder that your portrait files are going to go into for the particular custom portrait you are looking at - but that information isn't really necessary, because the "Open Portraits Folder" button will automatically open the right one.)
