You failed to read the OP, and no I did not. If that was the problem, I wouldn't have gotten the options the first time.
The thing is, just like the OP, I passed both the skill checks, but never even saw these options in my playthrough.
Succeeded all visible skill checks both times.

Yes, you can indeed approach from three sides. Tried them all, and even the side where I got the options the first time didn't give them to me on subsequent attempts despite successful rolls.
There are more than 3 sides to approach from.
There are two different ones from jumping down the pit and at least 2-3 more from different map screens. I can't quite remember and don't have a save to check.
I think it could be tied to that previous fight at the mines, where there was this boss, who was keep respawning at those lenses (sorry, cant remember the name). Back then I investigated those lense and when I was at this Rykad Minoris fight, it gave me an option to destroy lense and line was something like "You remember this thing from the mine boss fight, so you can shoot this thing". Fight was pretty hard to me, so I reloaded like 5 or so times with different angles and it had always gave me the option to shoot the lense.
The other question regarding this fight: what's the point of it? I completed it and destroyed the lense during initial prompt. Are there any short/mid/long term consequences? The loot was meh kind of level and I ddin't notice anything specific yet, and I am on chapter 3 now. Could miss though.
No spoilers, please. Rykad Minoris is the very first location I went to. I have no prior in-game knowledge of the lens, other than the shard in Theodora's quarters. The first time I entered the room, I got the option to shoot the lens through a successful knowledge (void) check.
I have no knowledge of any future consequences of the fight, but the room did contain information that the cult was aware of the player personally, and had been warned of our arrival. Also, there was some loot in the room, and clearing the fight cleared up access to the sewers, where further loot and a hidden fight with a herald of Tzeentch could be found, which rewarded additional loot and a trophy. And XP for the whole thing, of course.
Perhaps there is a hidden, difficult to pass check. I had this problem on my Noble, with the warp penalty. My other characters had no issues, weird.
Oops, pardon for spoliers. For some reason I thought that you go to that Minoris location closer to a chapter resolutions.
