and to your case with the operations: did you shoot the player or an explosive barrel next to him?
This was definitely friendly fire, but if the game isn't supposed to have it, and the last patch didn't patch it in either, then I assume this to have been a bug.
The two cases I know of are Krak Grenades and Melta Bombs. Krak Grenades can kill your teammates if a stuck enemy jumps at them just before it explodes, and Melta Bombs can kill your teammates if detonated via shooting (detonating them normally doesn't seem to TK).
At least it seems so as it does very little damage
Grenades (all of them) can deal friendly fire, I think on the 2nd and 1st difficulty you dont take damage from grenades, but once you enter the 3rd and 4th they will do damage
(these are rough numbers without any HP increases)
Frag will do roughly 50% if you stand in the middle
Krak about 80% in the middle
Melta will oneshot you if you stand in the middle (so be very careful with it)
Shock Grenades will do small but consistant damage over time, so dont stand inside it (the shock area Bulwark can create does NOT do damage while looking near identical, keep that in mind)
All grenades (apart from the Shock nade) do less damage the further you are away from the blast radius, in general, watch where you throw and blow your nades
And while I dont really have to mention it but feel like I still should (since in campaign they dont damage you yet in pve they do), enviornmental hazards like exploding flowers, red canisters and the blue power nodes that you can shoot so they explode
Not really sure how much damage they do but I believe they are similar to frag nades in terms of friendly fire, so be wary shooting them if someone's around them
