Operatoins Mode. Gene Seeds and Weapon Cosmetics not spawning?

The Gene Seeds and the Weapon cosmetics can spawn in multiple random locations but can these items not spawn at all? I've had a fair few runs where none of these spawn. I am wondering if it is a random chance to not spawn in the operation mode?!

There is a chance they might not spawn, as far as I've seen. On the higher difficulties they're much more common at least.
