genetials themselves don't, but correct/compatible gene expression does matter.
same with other body parts. e.g. if you lost your arm in an accident nobody will care, that's easy to fix. but if you were born without an arm that's a problem.
there's only two sexes: male and female, there's no ambiguity here.

As far as I know there are no female spacemarines yet but I think amazon was pushing for such.
A lot makes up your biology, a lot more than just genes. What I can think of spontaneously is regulatory sequences, epigenetics and of course you development which is insanely complex and can go wrong, including impacts from the evnironment. Genes are just a part of what results in you - even your DNA is more than just genes.
I can imagine issues during development can lead to gender expression not reflecting your chomosomes.
