How do you get equipment?

It would be nice if the update mentioned how you get equipment that carries over between runs to make the characters stronger but Im not finding any info other then seeing the icons for the gear on the map selection screen. I got about 18 minutes into a level but I didnt see any indicators on how to unlock the sets.

You are just not get blessed by RNGejus. Try to play on nightmare mode. I've got a dozen of copies of legendary sets here.

It drops from any enemy. Elites have 2x the chance, and bosses 5x the chance. The Sin events (purple) have a 100% chance to drop an item.

I just found one, but it's taking much longer than planned. I'll increase the chance and upload a quick fix shortly.

What was the fix? And how much does it increase the odds? I did several runs where I made it to night time and found nothing but then there was one round I found 3 pieces of gear at once.
