No you are not wrong, all the double holding points are missing. This I presume is due the scaling problems and the characteristics of the programming that for instance NB2W is either after or before N2W (Sorry can not remember which way round they are). Each holding point requires the program to use the holding point as a taxiway instead of a genuine Holding Point. The reality is that Holding points prior to a runway are indeed also taxiways so you are actually selecting a taxiway to hold at prior to entering the runway in the game.
Ironically, the developers have, at least for Heathrow added genuine holding points around the airport on taxiways but despite this haven't used the same thought process or even computer programming skills to make the entry points to runways both a taxi way and a holding point.
Because of this, and the obvious 'Scaling Problems', you will also see that aircraft can not pass other aircraft that are holding short of the runway as all aircraft no matter what the size they are block aircraft taxing on Taxiway A on both the north and south runways.
It would be nice if there were twin holding points one further forward than the other to facilitate aircraft passing on Taxiway A as it does in real life, but hey you never know, fingers crossed.
