No other way except exit game?

so my character lost both arms and is now trapped at the bottom of the toilet with 0 sanity and no way to die (maybe from hunger but i dont want to wait for so long). is it intentional? i thought something would happen when sanity reaches 0

It is totally possible to trap yourself in no win states, you just have to restart at that point. Your sanity dropping to 0 just causes you to permanently develop Panophobia, which causes you to take 50% more damage from everything in the game. The toilet specifically is just a trap to make fun of you for jumping into doo doo.

At the bottom of the toilet? Yeah, you're screwed, BUT if you ever find yourself missing limbs, have a piece of chalk and the Slyvian Skinbible, you can draw the sigil in one of those circles and it'll restore all your limbs. You just might have to crawl there, lol.

The toilet is the "curiosity killed the cat" situation that shoul tell you that not everything in the game warrants a reward.
That being said Termina, like many RPGMaker games, lets you return to the main menu by pressing F5
