This should either be a DLC or something completely crazy - like a free update.
As to why it's not for free: There's no such thing as a free lunch. These things cost a lot of time and money to produce.

- needs more (ANY) scope sway -- Sniper Elite is not Resident Evil
- even when sniper scope "glint" spotlights are on (should be 100% optional), they should only appear when the scope is aimed directly at the player, as distinct from just randomly popping up to give away enemy snipers' locations
- heart rate is currently meaningless beyond an on-screen gauge, and really needs a re-work to have an actual gameplay effect; it should also increase dramatically when climbing or hanging from a ledge, as well as when being shot at (suffice to say), and go well above 120bpm... these snipers are not all Bjorn Borgs
- bikes should not alert the player with "TOOT! TOOT!" every, single time they come within a certain radius of the player
- binoculars should not be long distance listening devices (not even Star Wars had that technology, and its tech. was all fairy tale wares) -- players' vicinity should dictate eavesdropping, even for mission critical dialogue exchanges
- binoculars should not measure distance down to the metre -- at best, to ±10m accuracy, decreasing in reliability with distance; and all "laser measuring" should be disabled for "Authentic" (or above) setting
- the X-ray needs to be tweaked to where explosions are not slower than the human's body being rag-dolled -- it makes no sense, and if the body moved that fast, it would disintegrate due to air friction; the body should move MUCH slower than the explosion effects and/or the shrapnel it flying from it
- from <200 metres away, in the dead of night, without any sound masking, or any suppressor attachment, enemies should spot the players unless they hide and re-position after each shot
- when shot at from distance, enemies should seek cover, not kneel down in the open to be executed, like some kind of virtue-signalling "BLM" cuck (as just as the such an ignominious end, in the context of said analogy, may be); they should also shoot from the hip when rushed by / rushing a player
- "Ribbon" notifications should be optional; at the most, display only for the first time they are achieved on a profile
- tools should be a load-out option, and take up a load-out slot irrespective of whether equipped or found in the game world -- if one cannot carry a special weapon on their back, they should not be able to carry a 10kg bolt cutter, or crowbar inside their rectal cavity
- unless on constructions site in the daytime, generators used for sound masking should be in the "off" state, to be started by players; especially so, when placed on night maps (I don't re-call the Krauts espousing the Chinese "9-9-6" work mantra)
- items found on maps should be placed in logical areas -- e. g., ammo in munitions deports, armories, weapons closets; medical aids in hospitals, bathrooms
- when enemies are shot without being killed, they should immediately yell, either for help, or in anguish (if hit), which should then alert any nearby enemies
- the reward for a successful active re-load should be the remainder of the current clip being kept
- when shot down at from the high ground, there should be a damage multiplier for the shooter above, in order to simulate the relative advantage of the vantage point... Anakin
- when an enemy is alerted to a grenade toss, they should jump for cover or hit the floor (assuming they do not throw the 'nade back at the player); not strike a pose in preparations for the X-ray death scene
- enemies should re-spawn after a time, so that the maps do not become unrealistically "cleared" as if it were score-based arcade game; ideally, "investigation teams" (elites) arrive after a time, in those areas where bodies have been left -- addressing both the aforementioned "cleared map" issue, as well as providing a reason (other than Survival mode clutter) to hide corpses
NB: I know this player had most, if not all of the helper sliders set to "Tricycle"; but, I wager that 90% of the points I made will be relevant across all difficult settings.
