I have been hit a few times now while no missile was within 4 km. The last time it happened I actually saw it "teleport" towards me.
My best guess is that when a plane whose current position is unknown fires a missile, the client acts like the missile has been fired from the last known position. But the server acts like it is fired from the real position. Then when the missile hits (or the plane who fired the missile becomes visible again?), the position is updated for the client, "teleporting" the missile.
