In the true ending, "he" does not mean Junji, means the blue hair boy. There're at least 3 parallel worlds in this game. But the main parts which you can get achievements are 2 parallel worlds. In the world A, 青空千绪A followed 明也A into the north alley and come out in world B. "he" means 明也A. He is missing. That's why 明也B needs to travel among worlds and refused 千绪B’s love.

If you know Chinese, you can check other posts about 4 hidden endings which are not recorded in the achievements. Also, I think I spent much more time on hidden endings than the normal endings. There're many hidden Easter eggs as well. Like the old lady in the park is actually 小满(千绪‘s best girlfriend) in world C. The blue bear is 千绪/player gave her in masako end (You need to collect 4 special coins to spin it in the machine in the convenient store, you can get a blue bear at the forth time and give 小满 in the park as a gift).
