[SPOILER] Doll disappearance bug?

Obviously, contains spoilers

After getting the red tape the doll disappeared, and now it is not appearing even on "resets" (quitting the current playthrough and starting over). Thing is, before solving puzzle to get the red tape I've already acquired scissors and decapitated her couple times, so I am not sure what is going on.

Is that an intended behaviour or a bug?

I assume I would need her head for further progress

Its intended. you just need to fully reset a run (get an ending) and she will appear again
