Missing scenes

So I played the game a few times with different choices but am still missing 2 scenes on page3, the first one (top left) and 5th (in the middle, row 2 column 2). Anyone knows how to unlock those scenes?

The one in the middle of page 3 is you stay at home at the end of the game rather than goto one of the other girls. In essence you are picking Mia. The one in the top left corner is giving me issues as well. I am not sure the choice path let alone whom it is supposed to be you are with. However with the scene progression it is someone between Yuki in the hospital with you and getting Ruth to blow you in the hospital. Maybe it is Sophie??

Got the scene in the middle. Only the top left one to find

The top left scene starts off the same as the second scene on that page. I followed the walkthrough so I don't know exactly how to get it, but I would think you have to focus on Ruth. Or you have to get foot jobs from some of the other girls. That's my best guess.
