You have to invest 1 skill point into any of these.
They can be charged to level 1, 2 or 3.
Each charge will add a better bonus, for example the bonus on the screenshot gives 2/4/6 seconds of paralyze effect on enemies.
Using them costs RAGE points. You gain RAGE points automatically each time you kill any enemy without using a powerup.

On a lot of games there is a charge bar. When the bar fills, the game gives you perks until the bar is emptied. On Van Helsing, you also have a charge bar, but it's shaped more like a vessel that is filled up (in the center of your UI), and it's called Rage, and when it fills up, you get to use the perks that come with having that filled up. The difference is that on Van Helsing, you get to choose which perks you will get, and how much attention will be given to each perk, and you can even choose when to use each perk.
However, if you don't want to deal with that much detail, you can simply choose your perks for each skill and let the game automatically use the perks when your Rage meter is full. This is what I do. I'm not a min/max player. I'm not looking for perfection in my setup. Whatever works is fine for me (most of the time), so I activate the perks that I want and let the game do it's auto-perk thing. Try doing that for a while until you understand the system better.
Also, in these Discussions pages, do a search on the word "Rage". The resulting threads will maybe help you clarify things.
