How to obtain monster lore

How do you obtain lore regarding monsters?

2 times has a monster individual dropped lore about themselves. So i have one tier of lore for both Draugr and Zombies, but is it completely random? Do you have to kill specific individual monsters? like named ones? or has it something to do with how many of that particular type of monster you have culled?

And another thing, and this was a problem in the original TSW also, enemies reacting to being hit is not synced with the animation of the player at all. this was always a problem and it's still here, kind of strange. not that it matters a whole lot, but it does look kind of weird.

the numbers telling me how much damage is being dealt to enemies pops up on the screen the second i press the button that activates a skill, and if that skill happens to kill them, they will just start their respective death animation instantaniously, even if my attack, be that a fireball or a bullet, has not connected with the enemy at all, this is kind of weird looking and makes the whole rythm of the game feel, well, just wonky and off. but i'll live i suppose.

i just wish that enemies reacted to actually being hit. i mean, i am firing a gun and the zombie is allready on the ground, deader, before the bullet leaves the chamber. dumb.

Lore for monsters is everywhere. Sometimes 1 or 2 pieces of lore drop in different areas from the monsters, but for the most part lore is just sitting around in the environment. If you want all of them you'll need a guide.
