Some questions about achievements

I'm trying to get 100% and I have some questions,
Firstly, can you get the larva achievement for Tri-achnid when you play in level select or do you need to get them all in a full run? Secondly, is there any way to save the progress of enter the unknown in Time ♥♥♥♥? Thirdly, do you need to complete all the levels to 100% meatboy or get all the bandages, or both? Also do you need to get A+ on them all?
