Map Stone - Path of Caradhras instead of Mines of Moria

I've played through the game and i'm hunting down achievements. And the map stones achievement is really weird for me. I have found all the map stones, but instead of the "Mines of Moria" map stone, is says "Path of Caradhras". What do you guys think, is there some way around it or could i start on a new slot and get it?

Yes I have. I have travelled from and to all the map stones.

Are you sure you have actually gone to each map stone in middle-earth and ACTIVATED it physically with a character?

Yes I'm 100 % sure!
I had to start another game and then it worked. So yes, I am 100 % sure that it's a bug.
