Yes, wellcome to MTGA.
The question is: Why would they return to casual niche formats, when they do not care for the community or its more casual members? WotC is great at many things and has been randomly generous over the years but at the same time they are bad at some fundamental things like creating and keeping community and listening to that community. Their overlord parent company keeps them at odds with this. The first thing they have to answer when designing new features in any of their products is: "How does this profit us and why would we bother when xyz works well enough?" 2hg while fun and probably a fan favorite amongst WotC staff, is too hard to get approval for on Arena when you take this question into account.
And likewise MTGO demonstrates a very basic implementation of Commander although it's pretty clunky and compromised by that interface/client, but still people made it work. Although I guess the better example is probably the unofficial versions of the game in games like Tabletop Simulator.
Honestly not sure that anything with regards to Magic is being made because it's a good business idea right now! But I would think that supporting literally the most popular mode of play (Commander) and a playerbase that loves to spend money on kitting out their decks is as close to a no brainer as anything in Magic. But we've been saying this for years and years...
And once you have Commander then 2HG just follows naturally since it can re-use the same interface. Definitely less demand but that's mostly because people have less exposure to it. If they played it they would realise how incredible it is, and how fun it is to play a coop team game mode in Magic. I always choose to play 2HG at pre-releases and I'm sure many others are the same.

Okay, when we here talk about "Multiplayer" in Arena or Magic in general, we refer to it being more than 2 players. 1v1 is the Generally accepted standard basic setup. Anything more than that is referred to as "Multiplayer."
Also, this is not a Card issue. This is a question of UI. Of whether or not the actual Client (ya know, the actual computer program that is running this digital game) could handle it. Yes, there are many cards that mention multiple opponents. But that's because Magic is a Physical Card Game before it is a Digital one. Playing with multiple other players is a simple matter in Real Life. Just need a bigger table, more chairs, and extra allotted play time. It takes a lot more than just a simple patch to do that in a Digital game. Especially if it wasn't originally set up to do so!
So yes, it is very possible, in fact likely, that Arena would need to be redesigned to allow for multiplayer (2HG, Commander, etc.) formats!
The story is that they think about it, but there is no active work on it. If you search for two headed giant on reddit for over the past year there's a small amount of discussion of it on there.
The devs did talk about this at a convention - the hope was that they might do something before 2025. Problem is that this was before they laid people off.
They realise that they are not going to build 4 player modes into the existing Arena client. The best you can hope for is a new desktop only client that uses the Arena collection. However, this new client would effectively be a new business unit, and so it would have to show a profit on a stand alone basis. They would need to completely overhaul matchmaking and chat - creating a need for expensive chat moderation that is largely absent from Arena.
My guess that it would only be viable as a subscription-only client. Random matchmaking is useless in a four player game - people will just drop out because they are whining about opponent's commanders, their initial draw, or because one player took 10 seconds to think about what card to play. If random matchmaking is not viable, FTP players are no longer needed to fill queues.
Or if you want to pay WotC (indirectly) to play Commander online (the main reason people want 4 player Arena), you can go play on MTGO.
