Is this during multiplayer or single player?
I just go for the NFL Rosters tab and sort them by age or position.
You only need two players for each position. Not including the cornerbacks and wide receivers.

If it's just vanilla stock and regular Franchise mode, something is definitely wrong. I would start a new Franchise and test to see if it happens there.
Correct... vanilla Franchise mode... it didn't start happening until later in the season, maybe like Week 14/Week 15... this led me to think it's the Progressive Fatigue, but when I disabled it the issue persisted. I never touched lineups or depth charts. Depth chart shows 1st stringers in their starting roles. They'll come in for the first one or two downs and be gone for nearly the remainder of the game, subbed in by their back ups.
Many defensive calls have "specialist" positions. Probably in the "specialist" tab you can move people around as you wish. Have never had any issue with getting who I want on the field.
It absolutely is progressive fatigue and you haven't paid attention to the player health option in weekly training.
You will see the see the accrual of fatigue in training every week, and can check their fatigue level in the rep setting for offense and defense.
Turning off progressive fatigue will not work mid season as it will not immediately refresh the players. It will only prevent additional accrual.
Your only solution at this point is to change Auto Subs from Manage Staff to reduce the levels at which the players are automatically subbed in and out. You could set it to Out 0 and in 1, for example, to sub out at 0 fatigue (depleted) and in at 1 (very low) but you might not like how your starters play with low stamina. May be better than your 2nd and 3rd stringers though.
Everything about the depth chart, stamina, and progressive fatigue work. If it seems like it doesn't work, you simply don't understand it. Granted there are hidden aspects like special teams which takes from your later depth chart positions in CB, WR, TE, etc.
