Got Glenn, Lugh and Urs. Missed Karren...
My guess is to get (not checked though):
Glenn - tell him you trust him when other guys doubt him in mines
Lugh - Go for kill all Mort when you can decide to kill or run/kill only leader + tell him you want to follow him when he wants to leave to do more killing (after map where he charges alone a bit), there was a conversation where he says he doesnt need the other guys but through a choice you can make him feel like an horrible person for saying that
Karren - not sure actually, at some point there is conversation where she's wounded and I think you need to stay behind to talk to her while helping with the wound
I stumbled upon this thread while googling about this during my playthrough (I play on playstation).
For anyone who might be interested:
After being at the end of the game I confirm that you can get them all in one playthrough.
It's pretty much same 236403mo listed. additionally: for Karren I think it was the "wait" option. Where Sarah says that Karren is wounded.
And for Urs, there is only one relevant conversation. I don't know the details anymore but I think it was the top option.
Choosing these options get you the number (2) info for each character. And after chapter 28, if you have unlocked everyone's (2) info, you have to choose top option 4 times (for every character) and you'll get the (3)'s as well.
(for the last part, It might be the case that you only get the option for each character if you have the (2) unlocked for that character in the first place. I had all the (2)'s, so I get 4 times a dialogue option for each character)
