Companions cannot die from starvation. So what's happening is that whatever they ARE eating is killing them.
Oki so like,,, They're trying to feed themselves and dying from it ?
They won't feed themselves if they're actively your companion however certain things like foods with the venom tag will just insta kill whatever eats it. There are other effects like poison or some apparently damaging skills from enemies that can sometimes say "starved to death" as the reason for death.
TL:DR though is that companions cannot starve to death. I currently have several starving companions in my party at every time because it's so expensive to feed 8 people at the same time.
This can be caused by being Wasted due to drinking too much booze or Cursed booze, or, for only the player and your followers, drinking or being hit by a Bottle of Contaminated Water.
Can you survive? Yes. You will however be missing out on a ton of things and there isn't a downside to creating one. You still have the pay the same amount in taxes regardless of if you have 0 towns or 1000 towns.
Eating spoiled or poorly cooked food while in the starving state is basically a death sentence.
Any food bought from a merchant "without poison, bleeding listed in the attributes" is safe to eat and will stay fresh indefinitely.
Food cooked with preservatives or made from jelly type enemies "putits . . . acid slimes" or golems will also be unable to spoil.
Buy cheap foods to stave off the hunger or buy better dishes to increase your followers stats. Starvation should be a rare occurrence.
Another thing to note, ANY food cooked or bought from merchants CANNOT SPOIL in your followers inventory, but it will likely be eaten so don't put your best food in there with the intention of retrieving it later.
Followers being ridden or riding you through the Symbiotic skill are also immortal and cannot die, but will go to sleep when their health hits 0 . . . which can immediately be reversed with any healing applied to them.
Alcohol or bad food has been making your followers kill themselves.
An exhaustive answer is this: If the material of the food isn't Raw Food, then it won't spoil. Cooked food from merchants is always Processed Food, but uncooked food they sell is Raw Food and thus can spoil (including Seaweed, which is made of Grass if you collect it yourself).
The only things I know of that can spoil that aren't made of Raw Food are ♥♥♥♥, which spoils despite it being made of Soil, and Bonito Flakes, only in the circumstance where they're in a Brewery Barrel.
