It's intentional. Random NPCs have random portraits. The same way we can encounter 37 year old children.

I dont like it. Im going to take a look at how to mod this game and restrict the random portrait selection.

Lol yeah I get your point, I was surprised too when I saw a child with granny picture. Even worse, that granny child wants to pay for my character physical acts.

The problem isn't randomness, it's sprite/portrait mismatch. It just looks dumb lol. I've gotten two people to buy the game and the first thing both of them asked was if their game was broken because of it. I'm sure someone will mod a fix for it.
