Had this happen to me, I would think he'd be guaranteed to appear given his presence, surprised he isn't
Also noticed I had a watcher kid missing on the same scene, had one sitting near the mouth-gravestones watching the show a couple times before, not now, might not be random and instead related to helping the kid on the underground of the city though
And one more while going through the flowery water tunnel on the way to the broken genie found a massive amount of lost village citizens, pretty sure I had no entities there before
Check if the condition for their appearance is the save file slot you’re playing on since that is a confirmed factor (the bus driver’s heads are cosmetically different and he gives different dialogue per file).

Sorry to say, but I'm confident that's inaccurate, same with people claiming taxi head is defined by the order you retrieve his head, I've had like 20 playthrrroughs at this point, like 15 of them on file 3, and I got to see all three heads, all while entering from the sleeve

They're so common to me I thought it'd be an anomaly to not have them
Its possible I didn't had them on my very first play but I wasn't sure if I just didn't notice, so didn't make a proper note of it
like the interactable one next to the interactable carboard cutouts? one that has the line about names being scratched out? theres also a non-interactable smaller one on the room before, I always saw them there on all my visits
ive only done the maze once but i also saw them there
